Hip pain has a way of sneaking into everything. It shows up when you climb out of the car, when you roll over at 2 a.m., and when you take the stairs at work. Where your hip pain is matters

Where you feel it is one of the best clues we have.

Outside of the hip. A sore spot over the bony point on the side of your hip, often worse when you lie on that side, frequently points to irritated tendons or a bursa, a small fluid-filled cushion. Front of the hip or groin. Pain deep in the crease where your leg meets your body usually involves the hip joint itself or the hip flexors.

Back of the hip or buttock. This one is tricky. Pain here often starts in the lower back, the sacroiliac (SI) joint, or a tight piriformis muscle near the sciatic nerve. The hip is just where you notice it.

The hip, pelvis, and lower back work as one unit, so a problem in one place tends to show up somewhere else.

Most hip pain does not come from one dramatic injury. It builds from small stresses repeated thousands of times. Here is what we see most around Southside and Deerwood.

The desk-and-commute hip. A desk job, a long drive on I-95 or J. Turner Butler, then an evening on the couch. Hours of sitting leave the hip flexors tight and the glutes lazy, and the pelvis can tip forward, loading the front of the hip and the lower back. The weekend athlete’s hip. Jacksonville is an active town, and that is a good thing. But repetition, like the twist of a golf swing or the quick lateral steps of pickleball, can irritate the tendons around the hip, especially when one side is doing more of the work.

The uneven pelvis. If one hip sits higher than the other, or the SI joint is not moving well, your stride changes. You may not notice, but your hip muscles do, and uneven loading is behind a lot of stubborn, one-sided hip pain.

The old injury and the worn joint. A past ankle or knee injury can change the way you walk for years, the same chain reaction we described in our knee pain guide. Arthritis in the hip joint itself also becomes more common with age. Chiropractic care cannot reverse arthritis, but improving how the surrounding joints and muscles move can make daily life noticeably more comfortable.

Simple things you can try at home

Before you book anything, a few small changes can take the edge off.

Get up every 30 to 45 minutes. Then do a gentle hip flexor stretch: half-kneel on a cushion and shift your weight forward until you feel a stretch in the front of the back hip. Hold 30 seconds per side.

Wake up your glutes. Lie on your back with knees bent and lift your hips into a bridge. Ten slow reps, once or twice a day, help the muscles that stabilize your pelvis.

Check your shoes, and pick ice or heat. If the tread is gone on one side of your sneakers, it is time for new ones. Ice tends to help a freshly irritated spot on the outside of the hip, while heat often feels better for general stiffness.

If the pain eases within a week or two, great. If it keeps coming back or keeps you from the things you enjoy, it is worth a closer look.

When the hip needs a doctor first

Get medical care right away if you had a fall and cannot put weight on the leg, if the hip is suddenly hot, red, or swollen, if you have a fever along with the pain, or if the pain is severe and getting worse at rest.
